Select the PCard Application and Cardholder Agreement Forms link from the Procurement Forms website (under PCard tab).
Your application will require accounting details to be associated with your PCard. Confirm the following details with the Business Office staff before submitting:
One application per card. If you will require multiple PCards, you will need to resubmit the form multiple times.
List Gina Tavis ([email protected]), HIDA PCard Manager, as the next recipient for the form when you submit.
